Many times, I see people posting threads thinking their fish has a problem and needs help but the description of the disease/condition they give is very vague or not understandable.

How about we do this, whenever you have a problem make a thread stating the problem as usual but this time, copy, paste and answer this questionaire after you state your problem so we can help you better. I got this idea from other forums

Basic Questions
------------------------

New additions/changes to the tank:

Unusual findings on the FH's skin:

Unusual behavior:

Medications/what has been tried or done do far:

Water Conditions
---------------------------------
Temperature (preferably in Fahrenheit):

Heater (yes/no):

-If YES type

Aeration:

Water change amount/frequency:

(the following answers need to be obtain by testing your water, if you aren't able to test your water, leave them blank)

Ammonia Level (ppm):

Ammonia tester used:

Nitrite (ppm):

Nitrate(ppm):

Are you testing with dipsticks?:

pH Level:

Tank Questions
----------------------------

Tank size:

How long has the tank has been in operation:

Type of Filtration:

Water additives/conditioners/pH adjusters:

What, how much and how often is your flowerhorn fed:

Tankmates (number, size and type of fish/other aquatic pets):



this way, we can identify problems earlier. So how about we all try this?

Also, someone needs to start a disease database so we don't get multiple threads on the same disease, and this way people can search them easier.

We can make a thread, and have people post the disease name, along with symptoms to look for and available treatment.

I think that is a wonderful idea... that way we can look up the different problems ourselves first...
